New patients can schedule an appointment by phone or online. Our staff is available to answer questions, assist with scheduling, and help determine the type of visit that best meets your needs.

What to Bring to Your First Visit

To help ensure a smooth check-in, please bring:

  • A valid photo ID

  • Insurance card (if applicable)
  • A list of current medications
  • Relevant medical records or referral information​​

Appointments are focused on understanding your concerns, reviewing your health history, and developing a plan of care tailored to your needs. Our goal is to create a welcoming, respectful, and patient-centered experience from your first visit forward.

Insurance & Payment Options

Most insurance plans are accepted, and self-pay options are available. Our team is happy to assist with insurance verification, coverage questions, and payment information prior to your visit.

Forms & Paperwork

New patient forms may be completed before your visit or at check-in. Completing forms in advance can help reduce wait times and streamline your appointment. Please send an email to info@accessfamilyclinic.com or info@accessbehavioralhealth.com to receive a copy of the forms. 

Telehealth Appointments

Telehealth visits may be available for select services. If you are scheduled for a virtual visit, instructions will be provided ahead of time to ensure a smooth experience.
